Call for current appointment rates.

I am currently not accepting insurance. If you choose to use your out-of-network insurance benefits, I am able to provide you with a “superbill” form to be submitted to your insurance company for reimbursement. Please be advised that your insurance company determines whether or not and how much you will be reimbursed. You are responsible for full payment at the time of your appointment.

Cancellation Policy

When we schedule an appointment, I reserve that time exclusively for you. A minimum of 48-hour notice is required for canceling or rescheduling an appointment. If an appointment is canceled or rescheduled with less than 48-hour notification, the full rate of that appointment will be charged.
